Ten Steps to Optimal Health with Healthy Aging Medical Centers

Below are ten steps to getting started with one of our treatment programs depending on which treatment program you decide to begin. Please follow all the ten steps to begin your program.

Comprehensive Initial Assessment

1. Get Educated. Explore our site, check out our resources page. Attend one of our interactive seminars. Next, call our office to have a preliminary discussion.

2. Start the intake process. Call Healthy Aging Medical Centers, Inc. (973) 325-6400 to begin the intake process or to ask any unanswered questions. You may also use our online registration process on our ePatientPortal at https://hamc.myemedfusion.com/epp.

3. Credit Card Authorization Form (CCAF). Click here to download our CCAF. Complete and fax to (800) 839-0396 as soon as possible to secure your appointment.

4. Health History Form. Click here to download our HHF. Follow the submission instructions on the form to submit this form.

Note: If prompted to allow macros while filling out this form in Microsoft Word please do so. They will not harm your computer and are necessary to complete this form.

5. Lab Tests (blood). After our office has received your CCAF and HHF, Dr. Rand will develop and forward a lab requisition to you by email. With that lab requisition, go to your nearest Quest or LabCorp lab to have your blood drawn. Present the lab requisition and the lab will forward the results directly to our office.

6. INITIAL APPOINTMENT. Your initial appointment with Dr. Johanan D. Rand, M.D. is specific to the program you are beginning. The HCG DIET program as well as the BIO-IDENTICAL HORMONE program includes a Comprehensive Health Evaluation. This appointment is up to 1 1/2 hours with Dr. Johanan D. Rand, M.D.. At the end of this visit, Dr. Johanan D. Rand, M.D. will discuss a customized treatment plan including very specific recommendations.  To learn more about the specific program you choose, please click on that program on our HOME PAGE drop down “Getting Started.”

Supplements

7. Order supplements and medications as necessary. All supplements are available through our online wellness store at www.healthyagingcentersstore.com.

8. Specialized Testing. When clinically indicated or otherwise indicated by lab results or other indications, Dr. Johanan D. Rand, M.D. may recommend specialized testing. This may include food sensitivity testing, toxic metals testing and/or nutritional deficiency testing. If you have agreed to proceed with specialized testing, we will provide the specific ordering instructions and any special requirements such as blood draws or other specimen collection.

Patient Notebook – Take control of your health care

9. When commencing a treatment program, patients are provided with a personalized notebook with placeholders for your specific treatment plan, lab results, educational materials and other information of interest. We also encourage and support the use of personal health records, such as Google Health or Microsoft Health Vault to further enable patients to take control of their health care.

Maintenance and Monitoring

10. Followup Visits. All of our maintenance programs include followup visits so that we can monitor and adjust your prescriptions and supplement recommendations depending on your body’s particular response to the therapies we employ. This component is the only way that Dr. Johanan D. Rand, M.D. can help you achieve the optimal health you deserve. Between visits, patients on maintenance programs have email and phone access to Dr. Johanan D. Rand, M.D. if issues or questions arise. The first followup visit is typically within 4 weeks for 12 month programs and the following week for 12 week programs. Patients are provided access to our web-based appointment scheduling system and are allowed schedule their own followup visits.
